Home Hospice North Lanark Seeks a Part Time Program/Volunteer Coordinator

Are you an RN or RPN? Are you seeking the opportunity to make a tangible difference in the lives of those living with a life altering diagnosis, who want to stay in their homes.  We also support those who are journeying through the last stages of life, the loved ones who care for them, and those dealing with grief and bereavement.

This position offers 20 hours biweekly on a one (1) year contract, which is renewable yearly.

The Role

The part time Program/Volunteer Coordinator reports to the Chair of the Board.

Program Coordinator Responsibilities

  • Conduct intakes and assessments and provide support for clients and caregivers.
  • Conduct comprehensive assessments of clients’ physical, emotional, social, and environmental needs to develop individualized care plans.
  • Monitor clients’ progress, reassessing their needs as necessary, and evaluating the effectiveness of interventions and services provided.
  • Maintain accurate and up-to-date records of client assessments, care plans.
  • Facilitate communication and collaboration among various healthcare providers, social service agencies, and community resources.
  • Provide public education and supports for programs. Attend Program Management Committee meetings, bi-weekly meeting with the Chair of the Board and monthly Visiting Volunteer meetings.
  • Provide ongoing support for full-time Program Coordinator.

 Volunteer Coordinator Responsibilities

  • Recruit visiting volunteers to the program as required. Coordinator would need to liaise with the Communications Specialist Consultant to advertise on social media as needed.
  • Screen all applicants and ensure that all required documentation is completed and on file.
  • Organize an interview with potential volunteers and the Board Chair.
  • Register applicants with Hospice Palliative Care Ontario (HPCO) for 35 hour required training.
  • Ensure that applicant receives a copy of all policies and procedures relating to the program
  • Register the applicant with Info Anywhere Software and provide orientation to the software with the applicant.
  • Match applicant with suitable client when all documentation, training and orientation is completed successfully.

Qualifications

  • Member in good standing with the Ontario College of Nurses.
  • Registered Nurse with education and experience in palliative/end-of-life/geriatric care, OR

A Registered Practical Nurse (RPN), with experience in palliative/end-of-life/geriatric care.

Requirements

  • Ability to collaborate with the multidisciplinary health care community to ensure that palliative support, founded on evidence-based best practices, is provided to individuals at home and across care settings.
  • Commitment to the importance of palliative care and teamwork.
  • Confidentiality, integrity and respect are key values.
  • Familiarity with grief support, and group facilitation.
  • Demonstrated effective leadership, interpersonal communication, and public speaking skills.
  • Experience working with volunteers preferred
  • Community nursing, and a solid knowledge of community resources preferred.
  • Availability during summer months and holidays and flexibility to meet the needs of the program.
  • Must have independent transportation.
  • If hired, the successful applicant will be willing to participate in Continuing Education such as Learning Essential Approaches to Palliative Care (LEAP) and complete the 35-hour Hospice Palliative Care Ontario (HPCO) training

Compensation  

  • In line with Leeds Lanark Grenville Public Health Nurses pay scale, commensurate with experience.
  • Reimbursement for mileage.
